WHO Bacterial Priority Pathogens List published: ID-CARE research team collaborates with AMR Division at WHO for 2024 update.

The WHO Bacterial Priority Pathogens List 2024 has now been published!

The ID-CARE research team, coordinated by Prof. Evelina Tacconelli, collaborated with Hatim Sati, the AMR Division of the World Health Organization and all the Advisory Group, for the 2024 update of the Bacterial Priority Pathogens List (BPPL).


This was a huge work that involved interactions between professionals from many different specialties, with a final output summarising the available evidence and providing guidance for the future research and development of new antibiotics. One of the aims of the update is to guide research and development for new, effective antibiotic therapies, aligning investment with clinical and public health needs.


See an overview of the list below, which now categorises 24 pathogens, spanning 15 families of antibiotic-resistant bacterial pathogens.


Gram-negative bacterial pathogens: a continued critical priority in 2024. 

Critical-priority group includes:

🔸 Acinetobacter baumannii carbapenem-resistant (CRAB)

🔸 Various pathogens in the Enterobacterales order (CRE and 3GCRE)

🔸 Rifampicin-resistant (RR) Mycobacterium tuberculosis (please read the full report for more details)


This update introduces a distinct ranking for 3GCRE, to highlight the need for targeted policies and interventions to address this emerging threat.


High priority group includes: 

🔸Pseudomonas aeruginosa carbapenem-resistant (CRPA) has been moved from “critical” to “high” due to a global decrease in resistance. However, investment in R&D for CRPA remains crucial!

🔸 Staphylococcus aureus methicillin-resistant (MRSA) remains a high priority in line with its high estimated burden.


The WHO BPPL ranking reflects a notable increase in recognition of “community” pathogens, indicating their growing resistance to treatments and the substantial burden they pose, particularly in LMIC:

🔸 Salmonella Typhi fluoroquinolone-resistant is placed very high among the community pathogens

🔸 non-typhoidal Salmonella fluoroquinolone-resistant was also moved up and is included in the high-priority category

🔸 Shigella spp. fluoroquinolone-resistant are other notable community pathogens that were ranked higher overall in the BPPL 2024 update, moving from “medium” to “high” priority.

🔸 MDR N. gonorrhoeae (including FQ-R and 3G cephalosporin-R strains) has also remained in the high priority


Medium-priority group includes:

🔸 Group A and B Streptococci

🔸 Streptococcus pneumoniae 

🔸 Haemophilus influenzae
